    Breed Survey February 12 2023
    BARRATT RESERVE WEST BEACH
    Commencing at 11.00am
    All entry fees & documentation MUST be received
    by 9PM on 1st February 2023
    Documents required are –
    1. Completed Breed Survey Application Form (GSDCA BS1)
    2. Five (5) generation pedigree
    3. Copy of ANKC pedigree
    4. Copy of HD & ED results
    5. Copy of Haemophilia results (if applicable)

    Please Note – Originals of the ANKC pedigree and HD & ED results
    MUST be brought to the survey and sighted by the surveyor.
    A blank BS1 form (1) is available via the club website and entries must be
    addressed to secretary.
    No Late entries will be accepted.
