    Documentation required:
    Completed BS1 Application form
    Copy of ANKC pedigree
    Copy of official HD and Elbow results
    Five (5) generation pedigree
    Copy of “Good” grading certificate or better.
    Copy of H-neg certificate if applicable
    Fee:- $35.00
    If any of the above are not received with the Application for
    Breed Survey the application will not be accepted.
